I got this glitch once while playing Ocarina of Time 3DS and died while fighting the Water Temple boss and I died. When I went back the music never changed to the boss theme. I have never anyone else mention this before but it did happen.

While playing medal of hoonour airbourne, I was starting the level (you start by jumping out of a plane with a parachute) and I somehow landed outside the level. I couldn't get into the level because there was an invisible wall in the way. Just as I was going to throw a grenade at my feet to kill myself and start again, I fell through the floor and died.
