Here, make an pokemon item that doesn't exist! Includes mega stones, or anything that's itemlike!
Bulletproof Vest- The following attacks do not work on the holder:
Shadow Ball

Restores the user's health a little and hurts the foe each turn.

Increases the power of dark type moves by x1.5 when held by a dark or ghost type.

The user never misses when holding this item.

Mega Blocker
No pokemon may mega evolve while the user is in battle, if the opposing pokemon has already mega evolved, it will devolve.

Death Ribbon
Triples the chance of the owner getting a Critical Hit if the owner is a Dark, Ghost or Fighting type.

Any Pokemon of the opposite sex is immediately attracted to this Pokemon
(especially effective on Nosepass and Probopass. Jk.)

The holder's sound moves can affect pokemon with soundproof and do 1.3x more damage on all pokemon.

Joy Buzzer
When the foe makes physical contact with the user, it may become paralyzed or confused.
Whoopee Cushion
If the user's Explosion or SelfDestruct attack fails to take the foe down with it, this item may expel another Explosion or Self-Destruct attack to finish off the foe.
Chatter Teeth
A wind-up chatter teeth toy that increases the foe's chance of flinching on a biting attack.

This item boosts the power of fairy moves. One use.

Life Vest
The Pokémon floats after getting hit by a Water-type attack.
(Like Air Balloon that only takes effect after a Water-type attack, only it doesn't pop)
Clown Nose
Adds a 10% chance of confusing the foe to attacks.
Surfboard
The holder's Water-type attacks will always strike first.
Gas Mask
The holder cannot be poisoned.
Hazmat Suit
Prevents status effects.
Chew Toy
Absorbs the foe's bite attacks.
Binoculars
When the foe tries to lower the holder's accuracy, it gets increased instead.
Rubber Duck
Activates Substitute when the holder is hit by a Water-type attack.
Fire Extinguisher
May leave the foe's Fire-type attack with no PP after the foe uses it.
Magic Broom
The holder may stay in the sky after hitting the foe with Fly.
Bubble Wrap
Halves damage from physical moves.

Generator
The holder takes 50% less damage from special attacks.

Alarm Clock- If the Pokemon is asleep, it will instantly wake up in between turns. Special property of this Hold item is you can only use it in 10 battles (including Wild encounters) and need to reset it after 10 battles, you must unequip it and put it in your bag and walk for 500 steps before it can be used again.

Toy Soldier
Increases the user's attack stat, but cuts accuracy.

Storm Stirrer
All moves that are pulse moves, moves affected by the weather or that involve wind (excluding whirlwind) have a 1.3x power boost and gain priority, but accuracy is cut slightly.

Guitar
All sound based moves' base power in increased by 1.3x.
Ice Cubes
Increases Ice-type moves by 1.3x. When the weather is sunny, the ice melts and the boost is lost.

Slingshot
Moves that strike more than once may last twice as long as they usually do.
(Bullet Seed 2-5 becomes Bullet Seed 2-10)

Snow Globe
Hail lasts longer (9-14 turns) and ice type moves are powered up by 1.3x when held.

Special Medicine
All moves that heal hp (damage dealing and non-damage dealing) heal the user slightly more.

Black Mask- Pokémon that are the Dark type have their attack stat doubled, but their accuracy falls by one. The mask cracks and breaks if it is hit with a physical attack over 80 power.
Gameboy- Foe may become confused or paralyzed if they hit the holder with a physical attack.
Mareep Wool- Halves damage taken from attacks, but sharply lowers speed.
That's all I got.

Basket- Allows the holder to hold two other items. Cannot hold two elemental plates or two of the same item.

Rubber Suit
An amazingly stretchy suit that stops Electric-type attacks, but Attack and Sp.ATK fall with each hit.
I can picture this being fun in Doubles, somehow. Give it to something with Contrary, maybe? Only other thing I can think of is slapping it on a Gyarados with Moxie, but not sure if that'd even work out well. Probably would work better on something defensive in most cases, anyway. Mantine?

Bird Cage
Opposing pokemon cannot switch out while the holder is in battle. Ghosts cannot switch out.

Majestic Antlers
These elegant antlers boost the attack of certain Pokémon (Deerling / Sawsbuck / Stantler) by two stages, but reduce the defense stats by 1 stage.

Masquerade Mask
Raises both defenses by 1, but lowers both attacks by 1.
Power Launcher
Raises both attacks by 1, but lowers both defenses by 1.

Magic Rune
When the user drops below 50% HP, this has a 30% chance of increasing Special Attack by 3 stages.

When the holder's health drops below 50%, this is a 30% chance of raising the holder's attack by 3 stages.

Seriously though, Sawsbuck and Stantler need way more help than Virizion.

Stopwatch
The holder gains +1 speed at the end of each turn. Same rules as speed boost.

Sharp Tooth
Increases your physical attack by 1.5x if your health is below half.
Now there could be a giant competitive use for this item if made.
Say Azurmarill came out and holding this item then used belly drum and got to half this would make Azurmarill incredibly strong or any Poke'mon that knew belly drum honestly.

Increases the chance of a move's added effect by 10%

Chakra
Any attacking move that is used from a distance hits twice, any move that already hits more than once can hit twice as many times.
Normal Move: 2 times
Repeating move (like bullet seed): 2-10 times

If held by Farfetch'd, increases critical hit chance is increased to 50%.
Something to make him more useful? But not really. v_v

Holy Monocle
Adds 50% power to moves that hit more than one pokémon, but they now only hit one target. Only works in double/triple battles.

This one's called the Arceus Amulet, and maximizes all stats, but causes confusion. It acts as a shout-out to the Alicorn Amulet form Magic Duel.
Now here's one called the Plenty Repel, and it lasts as long as your current session. It's useful if you want to just tour the region, and have an entire team of Lv. 100s, or want to hatch some eggs.

The Pixie Rod increases the power of Fairy Type moves.
The Glowing Spark increases the power of Light Type moves.
The Luminous Plate does the same, but this time it allows Arceus to become Light Type.

The plain old stick already does this, raises his crit rate by 2 stages to 3, which as of XY is a 50% chance to crit (or 100% if a high crit rate move), and unfortunately farfetch'd is still garbage xD

You only have to be one of the active pokémon when a mega evolution takes place. However, to make it not too OP, you can't switch in the pokémon holding the stone after the evolution and have its effect take place, nor can it be used if the pokémon is inactive/in your party. Also, forgot to mention it works on Primal evolution too.

Powershell
A sturdy shell that heightens def and sp def but lowers speed. Only works on turtle Pokemon
(also on side note, my Fakemon Voltortus evolves into Amportus or Surgortus based on its stats while holding Powershell on level-up)

This one is the Star Rod and is based off the Kirby weapon of the same name. It allows Yoshi to Primal Reverse, and is found in the VEEEEEERY center of the Prism Tower/Lumiose Gym during a REALLY long Fake Cheat involving the Odd Egg (which Yoshi hatches from), a decision question about whether or not you want to go with him, the Internet as an alternate dimension (you must have Nintendo Network and a stable Internet connection, as it asks you if you want to go online), Espurrs, the Psychic Amulet (how Yoshi gets his OP powers), and MLP-related things.

Makes every electric type move have +100 bp and Thunder always hits regardless of rain.

It's the Staff of Sameness, and the item I mentioned is the Staff of Normality, and it makes the opponent and his/her move types Normal, and if the holder is Starlight Glimmer, she gets to be Normal Type too!

Poisons non grass types, heals grass types by 6% of the holder's hp
[in other words, same as leftovers, but for grass types only, other types just get poisoned]

Fuzzy Tiger Doll
Use it to flee from a wild pokemon encounter, with a 20% chance to raise your attack or speed base stat by 1 and a 10% chance to catch the pokemon you faced without using a pokeball up!

Pixie Dust - Increases Fairy type moves
Glowing Orb - Increases Light type moves
Glow Plate - Same as above, but also changes Arceus into Light type
Quigon Berry - Reduces Light type moves that follow Light's contribution to the type chart (2x for Dark, Fairy; .5x Normal, Bug, Grass, Fire, Water)

Crystal Ball
Allows you to encounter any pokemon on the route you're on that is native to the route and the wild pokemon is unable to escape.

A hold item that restores 50HP if eaten.

Amulet (increases Psychic type moves by more than the Twisted Spoon, turns Yoshi into a Psychic type, allows him to grow wings when he reaches lv. 50)

Basic Box: If the holder is grass, fire or water it gets a random item.

Electro Lemonade
An item to be held by a Pokemon. When consumed in battle, it gigantically increases speed but lowers accuracy slightly.
Black Diamond Sword
An item to be held by a Pokemon. Slashing moves have their power increased by this item and it sometimes leaves the Wither status effect on the target.
Alola Stone
Converts a Pokemon to and from its Alola forme.
Firework Box
If the Pokemon is struck by a Fire-type attack or an explosion this box will explode on the attacker.
Speedblaster
A shoe to be worn by a Pokemon. It speeds the Pokemon up slightly and allows it to dodge quake moves more easily.
Ancient Claws
An item to be held by a Pokemon. It is a giant claw of a prehistoric dragon that powers up claw moves greatly.
Ice Bomb
An item to be held by a Pokemon. If the Pokemon holding it is attacked this bomb will explode and freeze all combatants solid.
Frozen Damp Rock
Extends the duration of the holder's Icy Rain weather.
Bathing Suit
An item to be held by a Pokemon. It is a cute bathing suit that powers up Water type moves and infatuates opponents hit by them.
Red Mushroom
A spicy mushroom that permanently changes you or your Pokemon to be male.
Blue Mushroom
A sweet mushroom that permanently makes you or your Pokemon female.
Black Mushroom
A disgustingly bitter mushroom. If eaten it destroys your ability to love and causes you to become permanently genderless.
Gold Mushroom
Makes a Pokemon shiny when eaten.
Bronze Mushroom
Causes a shiny Pokemon to become normal.
Silver Chocolate
Levels a Pokemon up by 50 levels.
Golden Chocolate
Instantly levels a Pokemon to Level 100 and PP Maxes all of its moves.
Bronze Chocolate
Sets a Pokemon to exactly Level 50. (cannot de-evolve a Pokemon)
Hustle Drink
Permits a Pokemon to attack twice in one turn.
Soul Sand
An item to be held by a Pokemon. It increases the power of Fire, Ghost, and Ground moves but prevents it from using Water, Ice, or Psychic moves.
Sea Lantern
An item that increases the power of Water, Ice, and Psychic moves but prevents Fire, Ground, and Ghost moves.
Platinum Crown
An extremely rare crown that sells for more money than you can carry!
Action Replay
Allows you to customize the game with various cheats and get event Pokemon and items.
Heart Needle
Increases the power of cursing moves and the power of the move Curse.
Cherish Ball
A special PokeBall for capturing Action-Replay-Only Pokemon. It has a normal catch rate for other Pokemon.
Legend Ball
A special PokeBall for catching Legendary Pokemon more easily. It is very rare.
Berserk Chromosome
When held by a Pokemon in battle its Attack power maximizes but it becomes male and gets a burn that cannot be healed.
Reflector Mirror
An item to be held by Wobbuffet. It enhances the power of its Counter and Mirror Coat.
Iron Armor
Adds the Steel type to the Pokemon holding the item.
Iron Hammer
An item to be held by a Pokemon. It powers up hammer-based moves such as Hammer Arm, Ice Hammer, and Sledgehammer.
Cursed Chocolate
An item to be consumed in battle. It is a piece of bitter dark chocolate that increases Attack and Defense but decreases Speed.
