Norm MacDonald did a good bit on this, where you don't really "lose" the fight with cancer so much as "it's a draw."
And furthermore, you don't wanna go out a loser that lost a fight with cancer.
Also, I don't see how people can really survive cancer on sheer force of will. It's really odd that this whole idea became a colloquialism.
